PROCEDURE FOR OUTPATIENTS

[Consultation by Appointment]
Except for emergency cases all consultations in all departments are conducted by way of appointment.
Please make an appointment in advance by referring to "HOW TO MAKE AN APPOINTMENT".

  1. First-visit to our hospital
  2. First-visit to a department that you have not visited before
  3. Revisit to a department last visited over six months before
  1. Revisit to your current department
  2. Revisit to a department last visited within the past six months


FIRST VISIT DESK (Desk No. @)
Come to the First Visit Desk (Desk No. @) at least 20 minutes before your appointment.
Inform the desk of your name and fill out our consultation form.
Present your health insurance card.
※ The full amount of medical fees will have to be paid if the card is not presented at the desk.
※ Please inform us if you were involved in a traffic accident or industrial accident.
Wait for us to prepare your hospital ID card and medical chart (karute).
Take the hospital ID and medical chart to the designated department.
AUTOMATED REVISIT COUNTER
Insert your hospital ID card into the machine, receive a numbered check-in slip and proceed to the designated department.

1.Present your hospital ID card and medical chart at the counter.
1.Place your hospital ID card and appointment slip (if one is available) in the box on the counter.

2.Please wait until you are called. We try to see patients in order of the number on the check-in slip.
※ Please understand that consultation may be delayed due to various circumstances.
※ Check-in numbers will be displayed on a screen in the waiting room for an approximate estimation of the time remaining until your consultation.
※ You can choose between receiving medication from the hospital or an outside pharmacy. Please consult your doctor.
※ You will be given a folder ("file") to take to the X-ray section, other examinations, or accounting desk (Desk No. F).
 
ACCOUNTING DESK (Desk No. F)
Hand in your folder. If this is your first visit this month, please also present your health insurance card.
It takes about 20 to 30 minutes to calculate your medical fees. Please take a seat and wait until your name is called.
Your name will be called from the CASHIER G desk.
X-RAY
If you are requested to take an X-ray, go down to the radiology reception counter on the B1 floor. Submit your folder and wait for instructions.
EXAMINATION
If you are requested to take any other tests (blood test, urine test, electrocardiogram, ultrasound), go up to the examination reception counter on the second floor. Submit your folder and wait for instructions.
   
CASHIER G
You will be called when your medical fees have been calculated.
Payment by credit card (DC, VISA, MasterCard, JCB, American Express) will be accepted.
After payment, you will be issued: (1) receipt, (2) next appointment slip and (3) a slip to receive medicines at the hospital pharmacy or a medical prescription for outside pharmacies.
Patients with a medicine slip for the hospital pharmacy
Patients who have a prescription for outside pharmacies
Patients with no prescribed medicine
PHARMACY J
When your slip number is indicated on the screen, present your slip at the counter to receive your medicine.
If you do not know how to take the medicine, please ask the staff behind the counter.

OUTSIDE PHARMACIES

Receive your medicine at a pharmacy outside the hospital.
